Registration for Confirmation

This form is for the parents of those who want to receive the Sacrament of Confirmation in Fall of 2027 at St. Thomas Aquinas Parish.

This year, sacramental preparation for Confirmation will include four stages of formation, totaling 22 one-hour sessions over a 12-month period.

Please Note:

Each student will need to schedule an individual meeting with Fr. Riley. More information will be sent in a follow-up email after registration.
If absent from a session, the student or parent is expected to contact Fr. Riley for make-up work.
- This sacramental preparation is intended for those who are currently in 8th grade (Fall '26).
- High-school students who have yet to receive the Sacrament in Confirmation are also encouraged to register.

PERMISSION AND DISCLAMERS

(My student’s name as indicated above) has my permission to participate in activities in the St. Thomas Aquinas and St. John Church and Student Center Parish Religious Education and Youth Ministry Program during the 2026-27 & 2027-28 school years. PERMISSION FOR DISCLOSURE AND EMERGENCY MEDICAL TREATMENT: The parish has my permission, in an emergency when parents or guardians cannot be contacted, to take my child to a hospital emergency room.
