2025-26 Confirmation Prep Registration

- This form is for the parents of those who want to receive the Sacrament of Confirmation in Fall of 2026 at St. Thomas Aquinas Parish.
  
  This year, sacramental preparation for Confirmation will include four stages of formation, totaling 20 sessions over a 12-month period.

  Each student will need to schedule an individual meeting with Fr. Riley.
  This sacramental preparation is intended for those who will be in 8th grade in Fall '25.
  High-school students who have yet to receive the Sacrament in Confirmation are also encouraged to register.

- PERMISSION FOR DISCLOSURE AND EMERGENCY MEDICAL TREATMENT: The parish has my permission, in an emergency when parents or guardians cannot be contacted, to take my child to a hospital emergency room. The hospital and its medical staff have my authorization to provide treatment that a physician deems necessary for the well-being of my child.
