InstructionsIf you are in need of a copy of your sacramental records from St. Thomas Aquinas or St. John Catholic Church, we will need the following in order to process your records request:A . The Sacramental Records Request Form to be completed and signed.Please print the form, review the Instructions box on the form before completing the form.B. A scanned copy of a photo ID for the person requesting the record must accompany the completed Request Form. (ie.Driver's License).C. Scan and send the completed PDF form and the copy of your ID via email to [email protected] and list "Sacramental Record Request" in the subject lineD. Please allow 2-4 weeks for a record request to be processed.Please Note:If you have a son or daughter and they are over the age of 18 and you are not their legal guardian, they are required to request their own records via the process above per diocesan protocol. (Ex. a Son and/or Daughter that is preparing for Marriage)We cannot process a record request from a parent on behalf of your son and/or daughter unless they are a minor and under the age of 18, or you have legal guardianship of the adult son/daughter.If you have additional questions, please contact Mariann Niznak at [email protected]
